ABSTRACT

ABSTRACT

Resultados: Dezessete prontuários integraram a amostra.

Aim: to evaluate the orofacial myofunctional condition of individuals with temporomandibular disorders, to characterize the mastication pattern and to investigate the relationship between the orofacial myofunctional condition and performance in mastication and swallowing functions. Materials and methods: medical records of participants in an interdisciplinary extension project in dentistry and speech therapy were analyzed. The minimum age to compose the sample was 18 years old. The participants had undergone a dental examination and speech-language pathology assessment, using the Orofacial Myofunctional Evaluation Protocol With Score protocol. Results were presented by absolute and relative distribution and measures of central tendency. Student's t-test for independent samples and Pearson's correlation were applied to assess association. The significance level adopted was 5%. Analyzes were performed using IBM SPSS Statistics 21. Results: seventeen medical records were part of the sample. The participants' mean age was 41.2 years (±12.2), 76.5% were female. The mean total score (OMES) indicated normal myofunctional condition (92.5±5.14), but mastication (7.94±1.60) and swallowing (13.1±1.17) revealed functional impairments. There was no significant difference between men and women in the total score (p=0.687), nor in the different categories evaluated. Discussion: The total scores found in this study, in mastication and swallowing, are in agreement with other studies carried out and reveal changes in function. Conclusion: functional variations found in subjects with chronic temporomandibular joint dysfunction in this study are related to general myofunctional orofacial disorder, and must be taken into consideration in treatment plans, with the aim of granting stability to the results.

ABSTRACT

Abstract Objective: The objective of this study was to analyze masticatory characteristics in children from 7 to 10 years, and to compare these characteristics among normal weight, overweight and obese children. Methods: This is a cross-sectional study, conducted in northeast Brazil, with a sample of 160 children, aged 7 to 10 years. The authors evaluated: nutritional and dental status; food intake; masticatory parameters and orofacial myofunctional characteristics. The children were divided according to nutritional status into normal weight (n = 101), overweight (n = 33) and obesity (n = 26) groups. Results: The results showed that Obese children had a larger bite size (g) (median = 6.0, Q1-Q3 = 4.0-9.0, p = 0.049) and performed fewer masticatory sequences (median = 3.0, Q1-Q3 = 2.0-3.2, p = 0.024) than children with normal weight (median = 5.0, Q1-Q3 = 4.0-7.0; median = 4.0, Q1-Q3 = 3.0-5.0). Furthermore, obese children finished feeding in a shorter time (s) (median = 62.5, Q1-Q3 = 50.5-70.0, p = 0.039) than normal weight children (median = 66.0, Q1-Q3 = 56.5-78.0) and overweight children (median = 66.0, Q1-Q3 = 58.0-81.5). Conclusions: The present results suggest that obese children present changes in mastication, evidenced by larger bite-size, performing fewer masticatory sequences and rapid mastication, which may contribute to increased food consumption and the development of excess weight.

ABSTRACT

Abstract Introduction Changes in breathing patterns affect the harmonious development of the structures of the craniofacial system, leading to changes in posture, occlusion, and facial growth patterns. However, little is known about how these changes influence the muscle contraction patterns, either at rest or while functioning, and either in a normal or unbalanced condition. Objective To study the masseter and anterior temporal muscles fatigue during mastication in nasal- and mouth-breathing children, also considering their facial growth patterns. Methods: A total of 70 children aged 6 to 12 years old who met the study criteria were assessed. Speech-language-hearing, otorhinolaryngologic, and cephalometric assessments were performed to divide them into groups. In the electromyographic assessment, the children were asked to chew gum following a metronome until they felt fatigued. The median frequency of the muscles was analyzed at 15, 30, 45, and 60seconds of mastication. The reported time of fatigue perception was recorded. The data were analyzed with analysis of variance (ANOVA) and the Kruskal-Wallis and the Mann-Whitney U tests. Results There were no median frequency decrease patterns nor differences in the myoelectric manifestations and reported time of fatigue between the groups. Conclusion The masticatory muscles did not reveal fatigue in the electromyographic analysis; however, the fatigue time was reported, despite the absence of physiological fatigue. The breathing mode, the facial growth pattern, and the association between them did not interfere with the behavior of the median frequency of the electromyographic signal and the fatigue time perception.

ABSTRACT

Abstract Introduction Clinical assessment in orofacial motricity is required for the speech therapist to diagnose and treat disorders involving the stomatognathic system. Validated tools can help establish a prognosis and outline intervention methods connected to human development. Objective The goal of the present study was to examine the domains of the oromyofunctional assessment of nursing infants and preschoolers according to sex and age group, as well as the application of the MMBGR Protocol - Nursing Infants and Preschool Children. Methods A quantitative technique was used to conduct an analytical and cross-sectional investigation. The present study included a total of 214 healthy breastfeeding infants and preschoolers of both sexes. The Mann-Whitney test was used to compare the medians. The Spearman correlation of each test domain was determined. R Core Team 2021 (R Foundation, Vienna, Austria) was used, and the significance threshold was set at 5%. Results In intraoral and extraoral examinations, there was a difference between sexes for tongue scores in nursing infants(d =-0.428; p = 0.045), worse in males. When the orofacial functions were considered in nursing infants, there were differences between the sexes for the liquid/solid/semisolid deglutition scores (d = 0.479; p = 0.031), with females performing worse. There were sex differences in solid/semisolid deglutition (d = -0.335; p = 0.043), and speech in preschoolers (d = - 0.478; p = 0.034), including the production of phones/phonemes (d = - 0.599; p = 0.007), which were always worse in males. Conclusion The research revealed sex disparities and related the domains of oromyofunctional assessment, according to scores, of the domains of myofunctional assessment, as recorded in a standardized oromyofunctional assessment protocol by age group.

ABSTRACT

Introduction: the etiology of malocclusions is related to genetic and environmental factors, and oral injurious habits are relevant in it; therefore, in a concept of integral health of children, we should count not only on the active preventive role of the dentist but also of the pediatrician. Method: to create a practical clinical guide for pediatricians that will allow the systematic identification of non-physiological habits linked to malocclusions. Result: the pediatrician with basic knowledge will be able to elaborate preventive check-ups during routine periodic check-ups. Discussion: consider multidisciplinary team care, with emphasis on prevention, a point to which the entire health team should contribute. Conclusions: we consider that the effects on growth and development caused by harmful oral habits/non-physiological functions will have greater or lesser repercussions depending on the age at which the habit begins. If we act early we will have more possibilities of modifying the growth pattern of the jaws and the development of the dental arches. Therefore, an early and systematic interrogation and functional examination is necessary to detect deviations and functional disorders in children, as well as to focus the treatments from multidisciplinary teams (dentist, pediatrician, speech therapist, otorhinolaryngologist, psychologist) and achieve excellent results.

ABSTRACT

Introduction: the stomatognathic system is responsible for innate and primordial functions for human beings, such as breathing, sucking, swallowing and phonation. The speech therapist is the qualified professional to carry out evaluation, diagnosis and rehabilitation of this system. Thus, such procedures are performed by speech therapy services, and this productivity is released on the corresponding platform of the Unified Health System (SUS) as "myofunctional assessment of the stomatognathic system". Objective: to analyze SUS investments for the myofunctional assessment of the stomatognathic system in Brazil, from 2008 to 2021. Methodology: this is an ecological study, of a quantitative nature, carried out with secondary data available on the DataSUS/TABNet platform. The data collected refer to values presented and approved for the myofunctional assessment of the stomatognathic system, in the aforementioned period, in Brazil. After collection, a descriptive analysis of the data was carried out, with exposure of absolute and relative values, growth rates and the deficit in transfers of financial resources. Results: in the studied period, 5.4 million myofunctional assessments of the stomatognathic system were performed, at a total cost of R$ 21.5 million, with growth rates of financial investments of up to 64.1% over the years. It was observed that, throughout the studied period, there was a deficit in transfers for procedures related to speech therapy services, reaching rates of 17464.6% of payments not made. Conclusion: it was found that there was an increase in SUS financial investments for the myofunctional assessment procedure. However, deficits in payments by the health secretariats were also noted, destined for a portion of the procedures performed in the speech-language pathology services.

ABSTRACT

ABSTRACT Dental prosthesis is the branch of Stomatology that is responsible for the study of the main principles of the rehabilitation of patients with dental absences, as well as for the prevention, diagnosis and treatment of alterations of the stomatognathic system. Qualitative research was carried out using theoretical methods-historical-logical, analytic-synthetic and empirical-, documental review, interviews with managing, academic and health care staff, residents and graduated. For its better understanding, the historical development was divided into three stages. The first stage describes the creation of health facilities, the beginning of undergraduate and postgraduate teaching, and also the creation of the provincial group and chapter of Dental Prosthesis of Sancti Spiritus, necessary to stimulate the scientific-technical development of the specialty. The second approaches the increase of services and the incorporation of new technologies for the comprehensive treatment of the specialists; and the third one the consolidation of the process of undergraduate and postgraduate training, which safeguards the care of patients with needs of rehabilitation treatments in the province. The specialty of Dental Prosthesis in the province has had an upward development in the province of Sancti Spiritus, and has a teaching staff strengthened for imparting undergraduate and postgraduate teaching. On the other hand, a steady training of the human resources has been kept, in correspondence with the needs of care of the population, and showing scientific-technical results that support the current work of the specialty.

ABSTRACT

Abstract Objective: To assess the effect of the three types of unilateral cleft lip (UCL) [cleft lip only, cleft lip and alveolus, and cleft lip alveolus and palate] on the outcome of the repair. Material and Methods: This study was a case series of effect of types of UCL on the outcome of the repair. Fifteen subjects each were in three UCL phenotypes groups. Evaluation of the scar, lip, and nose was done qualitatively by both parents/guardians and professionals using a modified form of the criteria described by Christofides et al. (2006). Results: In the assessment of the surgical scar, the parents found a difference between the three types of cleft in terms of texture, shape, and width of the scar and presence of columella deviation. The professional assessors, however, only found the three types of cleft to be different in the presence of alar flattening. Conclusion: Differences truly exist in the outcome of surgical repair of the three types of unilateral cleft lip, especially in the aesthetics of the nose and in the width and shape of the residual lip scar. Thus, it is important to consider this in the assessment of UCL repair because putting the subtypes together might have a negative impact on the assessment.

ABSTRACT

ABSTRACT Purpose: to verify, in the literature, the effects of using the stimulating palatal plate on lip and tongue posture in children with trisomy 21. Methods: a search was conducted in Medline, LILACS, CINAHL, EMBASE, Scopus, Web of Science, and Cochrane. Original articles designed as clinical trials, longitudinal studies, or case-control studies, approaching stimulating palatal plate in the treatment of children with trisomy 21 and assessing habitual lip and tongue posture as an outcome, were included. Literature Review: a total of 376 studies were found, of which 10 met the selection criteria. They were published between 1996 and 2007 and carried out mostly in Europe, with small samples. The age when they began wearing the plate ranged from 1 month to 5 years, and intervention lasted from 4 to 58 months; in most cases, it was combined with orofacial muscle stimulation. Use frequency ranged from two to four times a day, each period lasting from 30 minutes to 2 hours. The children's tongue and lip posture improved in most pieces of research. Conclusion: studies suggest that using the stimulating palatal plate in combination with orofacial muscle stimulation brings benefits to tongue and lip posture in children presented with trisomy 21.

ABSTRACT

ABSTRACT Purpose To Identify the occurrence of orofacial dysfunctions in young children and adolescents with acute leukemia who are undergoing remission chemotherapy. Methods Over a period of 16 months, 40 three to eighteen year -old patients with acute leukemia were admitted to the Amazonas State Hemocenter. Of these, 23 were included in the cross-sectional study and submitted to the evaluation of orofacial functions using the Nordic Orofacial Test-Screening, between D30 and D33 of the remission induction phase. The presence of oral manifestations was also evaluated via clinical examination. Results Orofacial dysfunction was observed in approximately half of the evaluated cases (n=11). Of these patients, all had alterations in the Dryness of the Mouth (VI) domain and 81.8% (n=9) showed alterations in the Chewing and Swallowing (IV) domain. Mucosites on lips, tongue, floor of the mouth and the oropharynx were the most commonly found oral lesions after the remission induction phase. According to the NOT-S, there was an association between the occurrence of oral lesions in the evaluated patients and the presence of orofacial dysfunction (95% CI, p-value = 0.027). Conclusion It is suggested that orofacial dysfunction is frequent in the remission induction phase in children and adolescents with acute leukemia. Studies regarding these orofacial dysfunctions in this population, as well as their relationship with oral lesions, are needed in order to fully understand their functional impact.
